6th Grade Application for the 18-19 School Year - Apply online starting Dec 4


Our updated 6th-grade application form will be available starting at 8 am on Monday, December 4th. Applications must be RECEIVED online by 4 pm on Wednesday, January 31, 2018. Applications will NOT be accepted at Spring Harbor Middle School. The MMSD Enrollment Office will email confirmation to parents when their application has been received.


A computerized student lottery will be conducted with preference given to:

  • Students living in the Memorial High School attendance area (students who live outside the Memorial High School attendance area will be added to the 6th-grade waitlist after the lottery is complete. Families residing outside of the Memorial High School attendance area must provide their own transportation).
  • Brothers and sisters of returning Spring Harbor students (for the 2017-2018 school year).

Ninety students will be admitted in 6th grade. All other students will automatically go on a waitlist.

A computerized student lottery will be conducted. Click on the Spring Harbor Lottery Process for the lottery criteria.

Parents will be notified by email of the results of the computerized selection process during the week of February 12th, 2018.

Lottery Application Timelines

  • December 4th - Lottery opens 8:00am.

  • January 17th - Open House at Spring Harbor.

  • January 31st - Lottery closes 4:00pm.

  • February 19st - Lottery results mailed to families this week.

  • February 28th - Accept or decline Confirmation due to schools from families.

Not currently a student in MMSD:

All Lottery applications need a student ID number issued by Madison Schools


To apply to Spring Harbor Middle, every applicant needs a Madison Schools student ID number. Proceed to the online lottery application if your student has a student ID number issued by Madison Schools.

Any student without a Madison School District issued student ID number, will need to complete the following abbreviated enrollment questionnaire before applying to Spring Harbor. Please provide a valid email address in the questionnaire.

Once the questionnaire is completed, the Madison Enrollment office will process the student information and issue a student ID number. The Enrollment office will email parents/guardians within 1-2 business days. The email will include the link to the Online application, the student ID number, and the parent/guardian's Infinite Campus parent portal username and password.
